В лекции рассматриваются вопросы цитологической диагностики патологических изменений железистого эпителия (ЖЭ) шейки матки. Описаны цитологические критерии категории AGC-NOS (атипичные железистые клетки без дополнительных уточнений), AGC-FN (изменения, подозрительные на неоплазию), AIS (аденокарцинома in situ) и инвазивной аденокарциномы эндоцервикса и эндометрия. Для каждой категории приведены ключевые цитологические характеристики – особенности архитектоники групп, ядер, цитоплазмы, фона препарата. Диагностика поражений ЖЭ остается сложной задачей, требующей высокой квалификации цитолога. Повышение точности диагностики возможно за счет использования жидкостной цитологии, стандартизированной классификации Bethesda, непрерывного образования специалистов и внедрения новых технологий.
